Volunteer / Service Hour Information

Below is the volunteer hour forms for the Senior 100 Hours Community Service, AVID Volunteer Form, National Honor Society, National Technical Honor Society and the PTA.  Seniors, Juniors, Sophomores, and Freshmen will be able to download these forms.  Volunteer hours & community service hours will be used for club requirements, scholarship opportunities, Senior 100 Service Hour recognition (this runs from the spring of the students’ Junior year to the spring of the students’ Senior year), and the PTA Presidential Volunteer Service Award (open to grades 9-12, students turn this form into the PTA).  We encourage each student to track their own hours and to be responsible for getting the information to their counselors, clubs, on college applications, and turned in for scholarship applications and awards.
